3) Choose the Items you wish to order and do a Final Recalculate on your Shopping Cart page. (Note - The purpose of the "Final Recalculate" is to give you an accurate total which reflects the Items ordered, the Tax, if applicable, and the Shipping Costs.)
4) Print your Shopping Cart Page (Note - This Shopping Cart information will NOT be sent to us, you must Fax it to us.
5) Fax us this Form and the Shopping Cart Page Printout to (859)225-5577. (Note - You will be faxing us a minimum of 3 pages and maybe more, depending on how many Items are on your List.)
